Health care coverage

Associates and their families are eligible for coverage under all company-sponsored health care plans on the first day of employment.

Medical coverage

Capital offers a variety of premier medical plans to choose from, including preferred provider organization (PPO) plans and at least one health maintenance organization (HMO) in most locations. Associates pay a portion of the cost for both PPO and HMO coverage. Associates may receive cash back by waiving coverage.

Dental coverage

Regardless of the type of medical coverage selected, associates may choose from either a low cost PPO dental plan (much like the preferred provider medical option) or, in most locations, an HMO dental plan (where associates agree to receive dental care from HMO providers only). Similar to medical, associates may waive coverage for cash back.

Vision coverage

Part of an associate's medical coverage includes enrollment in the Vision Service Plan (VSP). The plan helps offset costs of vision care, covering an annual vision examination, most lenses, eyeglass frames or you may choose contact lenses.

Spouse equivalent coverage

Same-sex spouse equivalents of Capital associates are eligible for company-sponsored health coverage.
